All students enrolled in Mounds View Schools must comply with the Immunization Requirements of Minnesota Statute and Rule. Students who do not comply will not be able to attend classes until requirements are met. Licensed School Nurses and Health Care Specialists will help families who need assistance to meet the immunization requirements in a timely manner.
Please note that requirements now affect students entering school sponsored Child Care Programs and Early Childhood Programs in addition to students entering Kindergarten and Seventh Grade. There have also been changes to the requirements for students entering Seventh Grade beginning in the 2014-15 school year. For students entering 7th grade, a Meningococcal and TDaP (Tetanus, Diptheria, and Pertussis) immunization are now required.

Dates of Immunizations may be provided to school from the child’s health care provider or noted by the child’s parent on the appropriate immunization form below.
Parents/guardians are encouraged to discuss any immunization concerns with their child’s health care provider. No student is required to receive an immunization if they have a medical contraindication, history of disease, or laboratory evidence of immunity.
For students having a medical reason why they should not be vaccinated the licensed health care provider must complete and sign Box 3A Medical Exemptions of the appropriate immunization form below.
Parents/guardians are encouraged to discuss any immunization concerns with their child’s health care provider.
For students whose parent/guardian has a religious or conscientious objection to immunizations the parent/guardian must sign and have notarized Box 2B Conscientious Exemption of the appropriate immunization form below.
