    Worker’s Compensation

    When a District employee is injured, immediately contact SFM Work Injury Hotline at (855) 675-3501. During business hours (7 a.m. to 4 p.m., Monday through Friday) employees can choose the report-only option (Option 1) if they do not need to speak with a nurse and just want to file a claim, choose Option 1. If the employee is not sure whether to get medical treatment, choose Option 2 to speak with a nurse. A nurse will provide immediate medical guidance depending on the seriousness and nature of the injury, and the best way to address it. Depending on the situation, the employee may be guided in first aid (“self-care”), allowed back to work, or referred off-site to a medical facility for further evaluation or treatment. Calls outside of normal business hours will automatically be answered by a nurse. 

    Whenever a call is placed, certain information must be collected to properly identify the employee and complete reporting requirements. This information is kept confidential and is only released to those who have a right to access it. Information is typically forwarded to Mounds View Public Schools Human Resources so it can be reviewed.

    Send a copy of your office visit summaries, report of workability, or any medical documentation related to your injury to Sandy Vue in Human Resources. If you miss any work time, notify Sandy immediately.

    If you are injured at work and are directed by the nurse hotline to seek non-emergency medical treatment, the following clinics are preferred providers who specialize in occupational medicine. Employees are not limited to these clinics and can go to any clinic of their choice.
