Early Childhood Screening

  • Minnesota state law requires all children to participate in Early Childhood Screening before entering public school kindergarten.  Mounds View PreKindergarten 4’s program also requires children to be screened, prior to starting school in September.  Screening determines if a child’s development is on track and provides families with information on community and learning resources for children.  

    Screening appointments are available at no cost to children ages 3.5 years old - 5 years old entering kindergarten in the fall.  The appointment takes approximately 1 hour and is located at our Early Childhood Education Center 500 10th St NW, New Brighton, MN 55112.  We are following the MN Department of Health and Education School Safety Protocols.  

    Please follow these steps:

    1.   Fill out the Child Health and Developmental History form by clicking here.
    2.   When we receive this completed form, we will send you an online assessment.
    3.   When we receive the results of your child’s completed assessment, we will contact you to schedule an appointment.  

    If you prefer all the forms mailed to your home to complete on paper, please call us 651-621-7350.

    If you wish to waive the early childhood screening requirement for your child, you can complete a conscientious objector form. Please submit to the Early Childhood office via email, mail or in person at Early Childhood Education Center 500 10th St NW, New Brighton, MN 55112.
