Early Childhood Screening

  • Minnesota state law requires all children to participate in Early Childhood Screening before entering public school kindergarten.  Screening can detect possible health or learning concerns, so that children can get help before they start kindergarten.

    During a regular school year, Early Childhood Screening would be available to families all year long and in person. However, due to the COVID-19 pandemic, we have adjusted our practices and procedures based on the MN Department of Health and Education recommendations this school year.  To ensure that children and adults stay safe, we are performing a majority of our screenings in a virtual format.   

    Screening appointments are currently prioritized to students enrolled in our program and incoming kindergarteners for the 2021-22 school year.  If your child is four years old or younger after September 1, 2020, you will be contacted for a late spring and/or early summer screening appointment.  Filling out the form below does not guarantee you an appointment.

    If your child turned four years old before September 1, 2020 and is attending pre-kindergarten in the fall, but has not yet completed an early childhood screening, please follow these steps:

    1.    Fill out the Child Health and Developmental History form by clicking here
    2.    When we receive this completed form, we will send you two online assessments.
    3.    When we receive the results of your child’s assessment, a screener will review all the information. 
    4.    We will contact you to schedule an online appointment.  Appointments will be 30 minutes or less and done via Google Meets or phone call with the screener.

    If you prefer all the forms and questionnaires mailed to your home to complete on paper, please call us 651-621-7350.

    Vision and hearing will not be screened during the virtual screening appointment.  If you have concerns about your child’s hearing or vision, please follow up with your child’s primary physician at your local doctor’s office.  If you have any developmental concerns about your child, please visit Help Me Grow and consider making a referral to early intervention.
