- Fill out the Child Health and Developmental History form.
- When we receive this completed form, we will send you an online assessment.
- When we receive the results of your child’s completed assessment, we will contact you to schedule an appointment.
For all other screening questions, please contact us at email@example.com or 651-621-7350.
If you wish to waive the early childhood screening requirement for your child, you can complete a conscientious objector form. Please submit to the Early Childhood office via email, mail or in person at Early Childhood Education Center 500 10th St NW, New Brighton, MN 55112.
